The Bolton Plan for Health and Social Care


On April 1, 2016, Greater Manchester became the first region in the country to take control of its combined health and social care budgets – a sum of more than £6 billion each year.  

Health and social care partners in Bolton will be implementing changes, gradually, over the next five years up to the end of 2021.

Bolton’s plan is all about ensuring that we have effective and sustainable health and care services in the future, as well as a happier and healthier population.  

The increased investment of £28.8 million for the first three years will enable Bolton Clinical Commissioning Group (CCG) to place a much greater focus on care in the community.

Over the coming years, the Bolton Partnership will be working to deliver significant changes to health and social care in Bolton.  We’ll update Bolton people along the way and keep asking for your views, ideas and feedback.


Our Plan for Health and Social Care – Useful Information

We will be joining up services and taking down barriers between teams and key workers to make the whole process much simpler - ensuring that people do not always have to attend hospital to receive the best possible care.

The initial investment, for 2017-18, will be £13.2 million which will be split between projects such as: Boosting community action, an expanded dementia service, improvements at GP practices, community-based assessments, a new mental health crisis centre, better access to talking therapies, preventing ill health and a technology boost for care homes.
